We are a professional pet sitting company that is available for 24/7 stays, overnights and drop by visits 365 days a year. We have been in business since 2005 with a staff that is bonded and insured. I have enclosed our Certificates of Insurance, one is for liability and the other is for our bond insurance. Both of these are for your records.
Please complete the following forms.

• Veterinarian Release Form, this would allow us to take your pet to your vet if you could not be reached and we felt that they needed medical care.
• Since you are going to give us a lock box, key or pass codes I will need the key release form filled out that gives us permission to have your keys or codes.
• Service Agreement explains all of our obligations to you as a pet sitter for your pets. Once you read and sign it please keep the yellow copy for your files.

The insurance and Bond Certificates are yours to keep.
